HOW TO: Accident Reports

To get a copy of an accident report

You may appear in person at the Precinct of occurrence and obtain a copy of your accident report up to 30 days of the accident date. You must bring id to recieve an accident report at the precinct.
Bring a  money order for $10.00 payable to N.Y.C. Police Department.
You must complete the Request for copy of AIDED/ACCIDENT RECORD

OR

If after 30 days, all requests must be mailed.
You must complete the Request for copy of AIDED/ACCIDENT RECORD which you can get from any precinct or on the webb at http://www.nydmv.state.ny.us/forms/mv198c.pdf
A  check or money order for $10.00 payable to Commissioner of Motor Vehicles
Enclose a stamped, self-addressed 9 1/2" x 4" envelope.
