Dear Ms. Coppersmith:

This letter confirms my November 21, 2005, visit to your 17228 N 16th Street, Phoenix, AZ facility concerning the General Liability and Property coverages.

PURPOSE OF VISIT

This visit focused on a discussion of your operations and a survey with John Melvin, Maintenance. I also met briefly with Gayle Swindle, HOA President

Based on my visit and observations, the following recommendations are being submitted for your corrective action. The recommendations listed below are intended to assist you in addressing physical condition(s) and/or Safety Management Practices which could result in incidents or other forms of preventable loss.

NEW RECOMMENDATIONS

05-01 IMPORTANT Fence And Gates Required For Pool

To help prevent unauthorized or accidental entry and subsequent injury or drowning, a fence, constructed in accordance with local building codes, should be installed around the perimeter of the pool area. Gates should be self-closing and provided with locks that should be secured at the close of pool hours. Gates should also be provided with devices that automatically secure themselves to the gate post when the gate is released.

The current pool fence is not tall enough.

Compliance is expected within 90 days.

05-02 GFCI (ground fault circuit interrupter) by Pool

The GFCI (ground fault circuit interrupter) protecting electrical equipment in the pool area should be tested once a month to make sure it is operational.


05-03 Roof Repair

The roofs should be evaluated and necessary repairs made by a licensed roofing contractor.

05-04 Lifesaving Equipment For Pool

To assist in the event of an emergency, lifesaving equipment including, but not necessarily limited to, the following should be placed in a permanent and readily accessible location:

a. A pool shepherd's hook or pole with blunt ends not less than 12' long.
b. A ring buoy of at least 15" in diameter or similar flotation device attached to a quarter inch throwing rope of a length equal to the width of the pool or 50'.

A shepherd's hook was provided but a ring buoy was not.

05-05 Depth Markers For Pool

To reduce the risk of injury or drowning from diving into shallow water, you need to inform users of the depth of water in the pool. Depth markers, in feet and inches, should be placed at or above the waterline on the vertical sides of the pool and on the deck near the edge of the pool. The markers should be numerals at least 4 inches high and placed at minimum and maximum points and at the break in the slope between shallow and deep ends of the pool.

Currently, markers are only provided at the water line and do not include the units of measurement.

05-06 Pool Deck Drains - Repair

Some of the plastic drain covers in the pool deck are damaged and should be repaired.

05-07 Walking Surface Maintenance

Properly maintained walking surfaces can reduce the potential for slip/trip/fall accidents.

Several areas were observed where drop-offs (change in elevation of several inches or more) were present at the edge of sidewalks. Where possible (such as drop-offs of only several inches at the edge of planters) these areas should be made more level (possibly by backfilling the planters.) For larger drop-offs (such as the one along the walkway on the west side of the building in the southwest corner of the property or the one at the sidewalk by the carport at the northwest corner of the cul de sac), consideration should be made to installing a guardrail or at least painting the edge with yellow traffic paint to make it more visible.

4. The parking stops separating the walkways from parking spaces (along the sides of the parked cars) should be painted to make them more visible.
5. Several areas were observed where the extended presence of water on a walkway has created moss. This should be corrected.
6. Lighting of many of the walkways does not appear to be adequate and should be improved to make the walkways more visible.
