reside in your Unit whether or not for a fee, a Tenant or Added Occupant will be viewed the
same. This application for new and renewal agreements is for:

In order to lease, rent or add an Occupant to Unit number _________of P.L.T.H., Inc. (PLTH)
the following is required:
• This application signed and notarized by the Unit Owners and Tenants (or added
Occupants)
• Copy of the executed lease, rental or occupancy agreement
• A check in the amount of $75.00 made payable to PLTH (Expedited fee of $100.00 if
processing required within 15 days of occupancy). Fee applies to each adult individual
that is not a family member.
• Occupational License for rental business, if required

Please complete and sign all appropriate sections of this application. The Board will not accept
applications from non-Unit Owners, partially completed applications or applications without a
notarized signature, applications not accompanied by the processing fee or applications from
Unit Owners who have outstanding Violations or unapproved Alterations.

and make further inquiry concerning this application.
I (We) acknowledge that the Board has the right to personally interview the Tenant(s) and
review with them the Governing Documents prior to approval.
If Occupational Licenses are not attached, I (We) acknowledge that I (We) have verified
with the City of Maitland and Orange County, Florida that an Occupational License is not
required for this lease or rental transaction.
I (We) acknowledge that I (We) have provided to the Tenant(s) or Occupant(s) a copy of
all current PLTH Governing Documents. If you are not sure, please contact the
Management Company for a current list.

I (We) further acknowledge that as Unit Owner(s), I (We) am liable for any damage to
common elements, Violation or unauthorized Alteration made by the Occupant(s) for
non-compliance with PLTH Governing Documents and Laws. I (We) further acknowledge
that I will be responsible for delivering any official notices or newsletters to the Tenant or
Occupant.
I (We) further acknowledge that all outstanding Violations, indebtedness and
delinquencies to PLTH have been resolved and that no Alterations have been made (in
accordance with the Governing Documents) that have not been approved and completed
or disclosed to the Tenant(s) and the Board.
I (We) further acknowledge that I have provided a key to the swimming pool or the $25.00
replacement cost for a new key.
I (We) grant PLTH a third party right of eviction in the event of a Tenant(s) or Occupant(s)
non-compliance with PLTH Governing Documents and Laws or if Tenants change or are
added without Board approval. In case of such eviction, all costs including legal fees will
be borne by me (us).
I (We) further acknowledge that the agreement is contingent upon Board approval. Upon
disapproval or eviction, any resolution of subsequent breach of any written or verbal
agreement terms prior to or after the agreement will be a matter solely between the Unit
Owner(s) and the Tenant(s) or Occupant(s).
I (We) acknowledge that the Tenant(s) named herein is (are) the sole Occupant(s) of the
Unit. Should that change I (We) will promptly submit an amended lease and application(s)
that must be approved by the Board prior to occupancy. Additional Tenants will require
an additional application fee for each unrelated adult Tenant.
I (We) are aware that any falsification or misrepresentation of the information contained
herein or refusal to provide information will result in an automatic rejection of this
application.

right to obtain a credit report or require additional forms of identification if necessary.

Unit before signing.
