?˜ Community Participation – leading a fuller life through meaningful activity
?˜ Community Presence – doing away with segregation by facilities, activities and schedules
?˜ Choices and Rights -- developing dignity and respect for one’s own choices
?˜ Competence – identifying what is important and obtaining any assistance needed to achieve one’s goals
?˜ Contribution – the focus person gives back something to the Network and to the community

Outcomes

?˜ An Action Plan that supports the focus person in becoming more connected within their community (school, work, neighborhood, congregation, etc.)
?˜ Puts everyone on “equal ground”
?˜ Answers the question, “Where do we go from here?”
?˜ Discovers more ways to successfully include the focus person
?˜ Creates or further enhances the vision for the focus person
?˜ Increases the quality of life through choices, empowerment, and by changing the way the person is viewed.
